Passport Information
Sahuarita Municipal Court has a passport agent available on Mondays only.
Hours for submitting your passport are as follows:
8:30 a.m. until 11:00 a.m.
1:00 p.m. until 4:00 p.m.
New Applicants: You MUST apply in person
1) Complete and submit form DS-11- Application for a U.S passport- Book or Card
A. You MUST apply in person
B. You MUST not sign the application until instructed to do so by the Acceptance Agent.
C. You MUST provide your SSN (social security number) in accordance with 26 U.S.C. 6039E
D. Acceptable/Valid identification must be presented at the time of application.
2) Submit proof of original U.S citizenship- Evidence of U.S. citizenship must be submitted with form DS-11. All documents submitted will be returned to you. Primary Evidence of U.S Citizenship:
A. Previously issued, undamaged U.S passport
B. Certified birth certificate issued by the city, county or state
Note-A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which MUST be within 1 year of your birth. Please note; some birth certificates are just a registration and may look like an original version of a birth certificate. These registrations will not be acceptable.
C. Consular Report or Birth Abroad or Certification of Birth
D. Naturalization Certificate
E. Certificate of Citizenship
3) Present Identification- When applying for a U.S passport in person, acceptable identification must be presented at the time of application. Primary Identification:
A. Previously issued, undamaged U.S passport
B. Naturalization Certificate
C. Valid Driver's License
D. Current Government ID (city, state or federal)
E. Current Military ID (Military and dependents)
Note- If you apply and submit an out-of-state primary identification, you MUST present a second ID containing as much of the following information as possible; your photo, full name, date of birth and the document issuance date.
4) Provide two (2) passport photos- Your photographs must be:
A. Identical
B. In Color
C. 2 x 2 inches in size
D. Printed on thin, photo-quality paper
E. Taken within the past 6 months, showing current appearance
F. Full face, front view with plain white background
G. Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head.
H. Taken in normal street atire:
- Uniforms should not be worn
- Do not wear a hat or headgear
- If you normally wear prescription glasses, a hearing device, wig or similar atricles they should be worn for your pictures
- Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons.
Renewal Passports- Renew by mail if all the following apply:
Your most recent U.S passport:
A. Is undamaged and can be submitted with your application
B. Was issued when you were age 16 or older
C. Was issued within the last 15 years
D. Was issued in your current name or you can legally document your name change.
Note- If the above statements do not apply to you you must apply in person.
To renew your U.S passport by mail:
A. Submit form DS-82
B. Two (2) passport photos (see section 4)
C. Copy of present identification
D. Submit expired passport
Note- Mail to the address on the form with all of the above.
Processing Times-
You should anticipate the processing of your routine passport application to take four to six weeks. If you have immediate travel, we recommend contacting the National Passport Information Center to upgrade your application to expidite.
